Know-by-Name is a system to address homelessness through a human-centered approach. It is grounded in the belief that every individual has the potential to thrive and achieve long-term stability.

Combining human support and relationships, personalized tools, and data-driven assessments, Know-by-Name (KBN) transforms how communities approach homelessness and increases the impact they can achieve. KBN is human-centered, evidence-based, and root cause-focused.


The Know-by-Name System at a Glance

Despite important advancements in collaboration and data-sharing, alongside substantial public and private financial support, Utah continues to see an increase in homelessness.

THE CHALLENGE

HOW IT WORKS

  1. Clients are assigned to a centralized case manager and complete a Pathway to Human Thriving assessment to identify a comprehensive set of needs across eight categories, with an associated status of Struggling, Surviving, or Thriving.

  2. The case manager and client co-create an Individualized Care Plan, setting SMART goals and coordinating across service providers to provide targeted support with specific needs. Case managers connect with service providers through weekly collaboration meetings to ensure that needs are being met and resources are used as effectively as possible.

  3. Clients tell their story once and then consent to their information and updates being shared quickly between case managers and service providers through a fully integrated HMIS system that is capable of both supporting clients and uncovering community-wide needs, trends, and insights.

  4. Clients receive continuous and personalized care throughout the process while measuring monthly progress. As care remains consistent, individuals move toward Thriving in all eight categories.

OUR HOPE FOR THE FUTURE

For individuals: Every client feels known and receives personalized, consistent, coordinated care as they advance toward long-term stability.

For communities: Deep coordination across all service providers creates personnel and fiscal efficiencies across the system. Advanced reporting, focused on outcomes (not just activities), identifies the organizations and programs that are most successful in driving long-term outcomes for clients.

The Utah Impact Partnership, an organization of Utah’s leading philanthropists, has been the exclusive funder and driving force behind the State of Utah’s Know-by-Name Pilot. Additional support has originated from the Utah Office of Homeless Services and MGT.
